Welcome to the Synapse reference guide, a resource developed to support GPs in their preventive practice during a time of high workload, time pressure, and information overload.
This guide provides an overview of the current, peer-reviewed scientific studies and international guidelines that form the basis of our algorithms.
The structure of this document mirrors that of the Synapse website. For each health domain, you will find an overview and explanation of the associated subscores, the biomarkers used in score calculation, and the underlying literature.

Digital health interventions have proven effective in driving behaviour change. Synapse is one such digital intervention, designed to equip clinicians with the insights needed to promote behaviour change and health improvement in their patients.
Behaviour change
The algorithms within Synapse are based on the most current medical literature. Risk categories are assigned to each biomarker based on the cut-off values described in that literature:
- No risk
- Moderate risk
- Elevated risk
- High risk
Each risk category is assigned a specific weight, enabling us to calculate overarching scores as well. Using these relative cut-offs, we arrive at a holistic estimate of the overall risk profile, allowing clinicians and patients to make better-informed decisions together.
